Responsibilities:

• Provide primary care services to a diverse patient population
• Manage chronic and acute conditions in an outpatient setting
• Order and interpret diagnostic tests
• Develop treatment plans and provide preventive care
• Maintain accurate EHR documentation

Requirements:

• APRN or PA License in the state of Connecticut (Preferred)
• Master’s Degree in Nursing or Physician Assistant Studies (Required)
• Experience in primary care, adult medicine, or urgent care preferred
• Spanish language skills preferred
